The airway problem with the beach chair position

okt 24, 2026

The beach chair position is frequently used for shoulder procedures, but placing a patient half-upright with their head fixed—and often rotated—creates a distinct problem for airway management.

High Leak Rates: Literature shows head and neck rotation leads to SAD air leakage in 3% to 20% of patients, depending on the device and angle (Lai et al., 2018).

Anatomical Displacement: Head rotation and extension compromise sealing function, alter glottic alignment, and lower oropharyngeal leak pressures across multiple supraglottic airway devices (Kim et al., 2019; Mishra et al., 2015).

In supine cases, a minor leak is often fixed simply by adjusting head position or reseating the cuff. In the beach chair position, your options are severely limited. Converting to endotracheal intubation mid-procedure on a seated patient with a draped, fixed head is far more stressful, complex, and high-risk.

The primary issue extends beyond leakage of SADs, but the lack of simple rescue options when the patient is upright.

LarynxLock addresses the root cause. By gently elevating the floor of the mouth, LarynxLock restores anatomical contact around the mask to maintain a reliable seal without requiring head repositioning or breaking the sterile field.

You don’t change your practice for the problems you can already solve; you change it for the ones where your standard fix is out of reach.
